Printing Request Form
If you are requesting permission from the Arts Education Partnership to excerpt an AEP publication or video, your organization must complete a Printing Request Form.

Please fully complete the form and fax it to the attention of AEP Staff at (202) 408-8081. If permission is granted the form will be countersigned and faxed to the requestor. Permission will not be granted over the phone.
